Post-hospital syndrome

We've long known that patients are at increased risk early following hospital discharge---for the first 30 days or so. What is less well appreciated is that only about a third of early readmissions are for the same diagnosis as the original admission. Rather, there appears to be a general state of vulnerability and poor reserve early after hospitalization irrespective of the original diagnosis. Hence the notion of post-hospital syndrome.
