1) It inhibits tubular secretion of creatinine. This results in a very mild rise in creatinine shortly after the beginning of treatment. There's no associated rise in the GFR. This is a benign condition.
2) Interstitial nephritis.
3) Hyperkalemia. It's an ENac inhibitor and this can have consequences, particularly in the elderly.
