I'm glad you find these posts useful. Most of the questions we answer come from our daily practice -- the staff at the perioperative center of Cleveland Clinic (IMPACT) sees around 13,000 patients each year. I expanded some of the cases during the last Perioperative Summit at the Clinic and they were published in the CCJM proceedings: http://www.ccjm.org/toc/2007periop.htm
